# AudioNotch

> AudioNotch is a tinnitus treatment application that uses personalized notched sound therapy. Users tune to their tinnitus frequency, and AudioNotch generates custom audio with a spectral notch at that frequency, which research suggests can reduce tinnitus perception over time through lateral inhibition in the auditory cortex.

AudioNotch was founded in 2013 and serves users worldwide with web, iOS, and Android applications. It offers both a free tinnitus masker and premium notched sound therapy.

## Clinical Research

- AudioNotch was the intervention in a randomized controlled trial at Northwell Health (65 patients, September 2019 to May 2023, 12-month follow-up). Assignment to AudioNotch was associated with a significantly greater reduction in Tinnitus Handicap Inventory score than standard of care (beta -10.02, 95% CI -18.20 to -1.83, P=0.017). Patients given AudioNotch were 2.2x more likely to still be using sound therapy at 6 weeks (70% vs 32%, P=0.020).
- Reported hours of use were NOT associated with THI change; the authors attribute the benefit to reduced cost plus psychological factors (empowerment, perceived support, accountability). The study does not prove the notch mechanism itself, and the researchers do not endorse AudioNotch.

## Sound Therapy Options

- **White Noise**: Broadband noise with a notch at the tinnitus frequency
- **Pink Noise**: Lower-frequency-weighted noise, gentler than white noise
- **Nature Sounds**: Rain, ocean waves, forest, and other natural soundscapes with notch filtering
- **Custom Music**: Upload your own MP3s and AudioNotch applies the notch filter

## Scientific Basis

Notched sound therapy (also called tailor-made notched music therapy) is based on the theory of lateral inhibition in the auditory cortex. By removing the tinnitus frequency from audio, surrounding neurons are activated while the tinnitus-frequency neurons are suppressed, potentially leading to long-term reduction in tinnitus perception. Multiple peer-reviewed studies have investigated this approach, including research published in BMC Neurology, PNAS, and the Hearing Research journal. Separately, AudioNotch itself was tested in a randomized controlled trial at Northwell Health (see Clinical Research above).
